Have you seen any of the binkie pod patterns. They are small and have a ribbon to attach to something. If you made it hang around neck, he wouldn't lose it. Or maybe use flurescent fabric.

You know, I'll bet if you thought about it a little, you could make the pouch in the shape of an ear. For the front, use a puffy batting under the material, and just quilt the outlines of an ear, you could trace any picture of an ear to get how it looks. Just a thought, hee hee hee.
